Student Health Insurance Plan (SHIP)

Mission

SHIP provides health care coverage for full-fee paying students and their dependents, seamlessly integrating campus primary care services with community specialty care, emergency services and hospitalization. SHIP is also an academic retention and learning support program that educates students on how to be savvy consumers of health care, focusing on how to effectively access and utilize all health-related services and insurance/financing options.

Eligibility

Effective fall semester 2003, Idaho State Board of Education policy requires full-fee paying students attending classes in Idaho to maintain adequate health insurance. Boise State University students registered for 8 or more credits (and all international students) are automatically enrolled in the university-sponsored Student Health Insurance Plan (SHIP), with the premium charge added to their tuition and fees billing. Students who provide proof of continuous enrollment in an alternative U.S.-based health insurance plan with comparable benefits are able to waive out of the SHIP coverage each semester. All students participating in intercollegiate athletics are automatically enrolled in SHIP. Requests for ICS students to waive out of SHIP must be requested and processed through the Athletics Coordinator Office. Students enrolled in SHIP are eligible to purchase coverage for their spouse and/or for any dependent children under the age of 19 who reside with the student.

NOTE: Part-time students are not eligible to enroll in SHIP. Part-time students may contact the SHIP office (208) 426-2158 to discuss their health insurance options.

Waiver Requirements

Students who provide proof of continuous enrollment in an alternative U.S.-based health insurance plan with comparable benefits are able to waive out of their SHIP enrollment. Waivers must be filed for both the Fall and Spring semesters. Deadline for fall semester is September 8, 2008 and for spring semester is February 2, 2009.

The test for comparability includes the following required components:

  • Alternate insurance plan must be with a United States based company.

  • The coverage cannot be voluntarily canceled and enrollment must be continuous through the current academic year.

  • The maximum policy limit can be no less than $75,000.

  • It is strongly recommended that coverage also include:

  • Full compliance with applicable federal law (specifically Title IX of Education Amendments of 1972, as amended by the Civil Rights Restoration Act of 1987). Specific components of these laws include pregnancy treated as any other medical condition.

  • Maximum of $1,000 individual deductible per year and standard co-payments not more than 50% of provider charges.

  • All full time and part time international students are required to carry the following additional coverage:

    1. Medical evacuation coverage must be at least $15,000.
    2. Repatriation coverage must be at least $10,000.

    If your alternative health insurance plan meets the requirements, please log on to BroncoWeb at broncoweb.boisestate.edu to submit your SHIP waiver request application. After you are logged in click on Students then Student Financials then Health Insurance Waiver.

    NOTE: THE WAIVER APPLICATION IS NOT FINAL UNTIL INSURANCE COVERAGE IS VERIFIED BY SHIP. STUDENTS WILL RECEIVE A NOTIFICATION OF APPROVAL OR DENIAL VIA BRONCOMAIL.

    Waiver Instructions

    If you have other, comparable health insurance and wish to opt out of the Student Health Insurance Plan (SHIP) you will need to complete the online waiver application. Have your insurance information available when completing the SHIP online waiver.

    Access your Broncoweb student account and to submit the Health Insurance Waiver Application:

  • Insurance Plan Name: Select the name of your insurance carrier from the drop down box. If your carrier is not listed, select "Other." (If you select "Other" you will need to type in your insurance company's name.)

  • Full Legal Name of Policy Holder: Insert name of policyholder. The Policy Holder is also known as a "Subscriber" or "Member" or "Enrollee."

  • Relationship: This is YOUR relationship TO the policyholder.

  • Policy or Group Number: Copy this information from your Insurance ID Card.

  • Member Enrollee Number: The Policy Holder, Subscriber, Member or Enrollee number listed on the insurance card.

  • Insurance Plan Contact Number: This is the Customer Service telephone number listed on the insurance card.

  • Click "Continue."

  • READ CAREFULLY THE NEXT PAGE. Click "Continue."

  • PRINT THIS PAGE and RETAIN COPY FOR YOUR RECORDS.

  • THIS IS YOUR ONLY PROOF THAT YOU HAVE SUBMITTED YOUR WAIVER.

  • If you have any questions regarding the waiver instructions, please contact the SHIP Office at SHIP@boisestate.edu or 426-2158 prior to the waiver deadline.

    Please note: The waiver process is NOT COMPLETE until you have printed out a copy of the receipt for your records. Failure to complete a waiver will result in automatic enrollment in SHIP.

    SHIP Tips

    All services performed off campus are subject to deductible and co-insurance, this includes any laboratory tests drawn or collected at the Health and Wellness Center and sent outside for review and/or interpretation (e.g, pathology, other labwork that is not included in the SHIP wellness benefit etc).

    The student health insurance coverage is with The Maksin Group. Coverage is effective August 1, 2008 and ends December 31, 2008 for ICS Athletes and August 15, 2008 and ends January 14, 2009 for Academic Students for the fall semester. Spring enrollment is effective January 1, 2009 and ends July 31, 2009 for ICS Athletes and January 15, 2009 and ends August 14, 2009 for Academic Students.

    The maximum benefits on your policy are $100,000, unless you add additional catastrophic coverage. Catastrophic coverage is available to purchase during the fall open enrollment period only.

    Wellness benefits are only covered in the Health & Wellness Center on campus.

    Maternity care is a covered as any other accident or illness. Chiropractic services are limited to 24 visits and Physical Therapy services are limited to 20 visits per policy year. Please find an in network provider to obtain the best benefit level.

    Students at BSU may add dependents to their insurance policy by logging onto wwww.maksin.com/bsu.shtml or call 877-775-5430.

    Dependents are not eligible to receive services at the Health & Wellness Center on Campus, therefore, all dependents are subject to the deductible and do not have routine/wellness benefits. For alternative plan information please contact the SHIP department at SHIP@boisestate.edu or call 426-2158. Dependent coverage is handled entirely by Maksin.

    If you need medical attention when Health & Wellness Center is closed you may go to the nearest Primary Health (Broadway and Garfield) or select any in network Urgent Care. If this is an emergency call 911 or go to the nearest emergency room. Charges will be subject to deductible and coinsurance.

    If you have a claim that is not being paid, please contact your insurance company immediately. Do not assume billing problems will "go away". If you require additional assistance, the SHIP department is available Monday through Friday.
