Student Health Insurance Plan (SHIP)
Mission
SHIP provides supplemental health care coverage for full-fee paying students and their dependents, seamlessly integrating campus primary care services with community specialty care, emergency services and hospitalization. SHIP is also an academic retention and learning support program that educates students on how to be savvy consumers of health care, focusing on how to effectively access and utilize all health-related services and insurance/financing options.

Eligibility
Effective fall semester 2003, Idaho State Board of Education policy requires full-fee paying students attending classes in Idaho to maintain adequate health insurance. Boise State University students registered for 8 or more credits (and all international students) are automatically enrolled in the university-sponsored Student Health Insurance Plan (SHIP), with the premium charge added to their tuition and fees billing. Students who provide proof of continuous enrollment in an alternative U.S.-based health insurance plan with comparable benefits are able to waive out of the SHIP coverage each semester. All students participating in intercollegiate athletics are automatically enrolled in SHIP. Requests to waive out of SHIP must be requested and processed through the Athletics Coordinator Office. Students enrolled in SHIP are eligible to purchase coverage for their spouse and/or for any dependent children under the age of 19 who reside with the student.
NOTE: Part-time students are no longer eligible to enroll in SHIP. Part-time students currently enrolled in SHIP should contact the SHIP Office at (208) 426-2158 to discuss their continuation or conversion options for this semester.

Students enrolled in SHIP are eligible to enroll in the following optional coverage plans:
Dependent Coverage
Students may enroll their spouse and/or dependent children under the age of 19 currently living at home. Please refer to the premium rates for cost of available coverage. All dependents added to SHIP have coverage for illness and injury. Preventive care is not a covered benefit for dependents. All dependents are subject to the deductible per academic year. Dependents are not eligible to receive medical care at the Health, Wellness and Counseling Services on campus. The SHIP office has a list of preferred provider clinics available in the Ada and Canyon County area for primary health care. Catastrophic Coverage
This plan extends your maximum benefits allowable from $100,000 to $500,000. Catastrophic coverage is available to both students and their dependents. Enrollment is annual and only offered during the fall open enrollment period which is 30 days before the fall waiver deadline. Please refer to the rate guide for costs. download the form (PDF)
Co-Health Group Collegiate Plan
This plan provides discount care for dental, vision, and pharmacy coverage, and is available for annual enrollment during the fall open enrollment period. This is not an insurance plan but offers discounts for vision, dental, and prescriptions with contract providers. This plan is available to both students and their dependents. Please refer to the rate guide for costs of coverage. More information can be found at www.cohealthusa.com.
Waiver
Students who provide proof of continuous enrollment in an alternative U.S.-based health insurance plan with comparable benefits are able to waive out of their SHIP coverage each semester. Waivers must be filed for both the Fall and Spring semesters. Deadline for fall semester is September 1, 2006 and for spring semester is January 29, 2007.
The test for comparability includes ALL of the following required components:
- Alternate insurance plan must be with a United States based agency.
- The coverage cannot be voluntarily canceled and enrollment must be continuous through the current academic year.
- The coverage must be in full compliance with applicable federal law (specifically Title IX of Education Amendments of 1972, as amended by the Civil Rights Restoration Act of 1987). Specific components of these laws include pregnancy treated as any other medical condition.
- Maximum policy limit can be no less than $75,000.
- There can be no more than a $1,000.00 individual deductible per year and standard co-payments can be no more than 50% of charges.
- All international students full-time and part-time must be enrolled in a U.S.-based medical plan.
- Medical evacuation coverage must be at least $15,000.
- Repatriation coverage must be at least $10,000.

SHIP Tips
All services performed off campus are subject to deductible and co-insurance, this includes any laboratory tests drawn or collected at the Health & Wellness Center and sent outside for review and/or interpretation (e.g., pap smears, cholesterol screening, etc)
Beginning Fall 2006, insurance coverage is with UnitedHealthcare. Coverage is effective August 15, 2006 and ends January 14, 2007. Spring enrollment is effective January 15, 2007 and ends August 14, 2007.
The maximum benefits on your policy are $100,000, unless you add additional catastrophic coverage. Catastrophic coverage is available to purchase during the fall open enrollment period only.
If you require services from a specialist in the UnitedHealthcare network, a $250 deductible will apply for services rendered outside of the campus health center. If you choose to use a non-network physician, an additional $500 deductible will apply for services performed outside the campus health center.
Maternity care is a covered benefit as any other medical condition. Please make arrangements with your attending physician’s billing department when you schedule your initial visit to discuss finances.
Chiropractic and physical therapy will be covered at maximum of 24 visits per policy year and only if performed by a network provider.
Students at BSU may add dependents to their insurance policy. However, dependents are not eligible to use HWC for their medical care, therefore, all dependents are subject to the deductible.
The insurance claims center will not process claims as a result of an accident until accident information has been filed. Download the medical claim form (PDF).
If you have a claim that is not being paid, please contact the SHIP office immediately. Do not assume billing problems will "go away". The SHIP office is available Monday through Friday to help with insurance questions.
