Integrity Hotline Reporting Form


What You Can Report

Any activity by Boise State University or a Boise State employee that violates any state or federal law or regulation (e.g., corruption, malfeasance, bribery, theft or misuse of government property, fraud, coercion, or conversion) or University policy; or wastes money, or involves gross misconduct, gross incompetence, or gross inefficiency.

Where To Report

  • Your supervisor (or other appropriate administrator within your unit), who will report it to:
    • Human Resource Services or
    • Internal Audit & Advisory Services or
    • University Counsel or
    • Risk Management & Insurance
  • Directly to any of the above offices
  • Boise State University Integrity Hotline at 208-334-2461

Personnel matters should be addressed with Human Resource Services (426-1616) directly. Discrimination and harassment complaints should be raised with the Affirmative Action Office (426-1979) for investigation and problem resolution. Student/faculty complaints should be forwarded to the Student Conduct Officer (426-1583) or ASBSU Academic Grievance Board (426-1440).

How To Report

  • In writing, please complete the form below electronically or return to Mail Stop-1240 or physical location 1117 Denver.
  • In person, visit with one of our auditors in Internal Audit and Advisory Services located at 1117 Denver.
  • Via the telephone, call the Integrity Hotline at 208-334-2461 and leave a message 24-hours a day with as much specific factual information as possible (report what you know, but don’t investigate – that is not your responsibility), or speak with an auditor directly by calling 426-1533.
  • Anonymously, if preferred

Confidentiality will be maintained to the highest extent possible

Protection from Retaliation

If you believe you have been retaliated against for whistle-blowing, you may file a complaint with Human Resource Services or your supervisor.
