All these forms are in Word 97-2003 Format. Event requests should be sent via e-mail to prkevntreq@boisestate.edu. Appeals forms should be sent to via e-mail to prkappeals@boisestate.edu.
To do this: Click File → Send → File as Attachment.
The addressee as well as the above instructions are noted on each of the forms.
If you are on the Boise State Campus the above will not work. You will need to save the document to your hard drive and manually attach it to an email to prkevntreq@boisestate.edu or prkappeals@boisestate.edu.
