Form Examples
Currently, the only form being run through IPT is the Agency Affiliation Agreement. As more forms are posted to the system, this page will be updated to provide examples and brief explanations. The BSU School of Social Work will also communicate any new IPT features via e-mail, so please keep your information current.
Agency Affiliation Agreement
This Form serves as BSU's official agreement between the university, the agency, and the student. The Affiliation Agreement lists the responsibilites of all parties during a set learning contract period. The student will first read and sign the Form via IPT. After the student signs their portion, IPT willl automatically e-mail you as the supervisor to let you know that there is a Form waiting your action. You then will login to IPT, click on the My Forms link, and then click on the View link to read the agreement. The Form requires you to fill in your agency name, the dates (starting and ending) of that student's practicum term, and at the bottom of the Form to fill in your name, the dates, and then electronically sign the document. Be sure to hit the save button before leaving Form or your changes will not be saved. After signing the Agency Affiliation Agreement, IPT will send the Fieldwork Director a notice saying that the Form needs to be completed. Once the student, the field instructor, and the Field Director have signed off, the Form is then complete. You will always have access to this Form if you want to print it out for your records.
