Form Examples
By Fall 2008, all Practicum forms will be circulated via IPT. As more forms are posted to the system, this page will be updated to provide examples and brief explanations. The BSU School of Social Work will also communicate any new IPT features via e-mail, so please keep your information current.

Before any student initally meets with the Practicum Director, they must complete the Field Practicum Application. This form will ask you to fill out a number of questions to understand your needs and interests regarding your placement. After filling in all the fields, remember to sign the form and click the save button at the bottom of the form or else your information will not be forwarded to the Social Work department. Please read all information on the Application to be sure you understand the requirements of your Practicum.

This form confirms your practicum placement. Fill this form out only after you have met with the Practicum Director and then met with the Field Agency. After the Agency agrees to take you on as a practicum intern, this needs to be filled out in its entirety. The forms asks you to provide your contact information, contact information of the Agency, and contact informatino of your Field Supervisor. The School of Social Work will then take this information and update your record and make any needed updates to both the Agency and Supervisor record. Again, please read the entire form to be sure you understand all of the stipulations of your internship. If you have any questions, please contact the School of Social Work.

Agency Affiliation Agreement
This Form serves as BSU's official agreement between the University, the Agency, and the Student. The Affiliation Agreement lists the responsibilites of all parties during a set learning contract period. The student will first read and sign the Form via IPT. After the student signs their portion, IPT willl automatically e-mail the supervisor to let them know that there is a Form waiting their action. The Form requires you to fill in your name at the bottom of the Form to fill in your name, the date, and then electronically sign the document. Be sure to hit the save button before leaving Form or your changes will not be saved. Once the Student, the Field Instructor, and the Field Director have signed off, the Form is then complete. You will always have access to this Form if you want to print it out for your records.
