This form is only for 2nd degree seeking students who have been admitted to Boise State.

Your checklist request should be submitted 4 weeks prior to the start of the term you wish to enroll in. Requests submitted after that deadline may not be processed in time to start classes. Please only submit a request after completing all second-degree admission requirements.

If you have previously attended Boise State University, COBE Advising can access your transcripts using your student ID number. If you have attended a school other than Boise State, you will need to upload your transcripts for evaluation. (The department cannot advise you on which courses you need for a second degree without seeing which courses you've already completed during your first degree.)
