The Minors on Campus program has purchased an accidental medical policy on behalf of all University-operated youth programs with coverage for participants of that program. For coverage to apply, the youth program must be registered through Minors on Campus.
The purpose of the insurance is to assist with medical expenses for participants who have sustained an injury requiring medical treatment while participating in a Boise State youth program. The policy coverage is offered without consideration of fault, negligence or liability and should not be construed or presented under those considerations.Â
The coverage is excess to any primary health insurance coverage available for the participant as an insured. It will act as primary insurance, should the participant not be covered under any primary health insurance policy.

General Claims Protocol
All injuries sustained by participants enrolled in University youth programs will be reported by program staff using the Incident/Accident Form: Youth Program Incident/Accident Report.
Program Directors or program staff are responsible for reporting all injury incidents/accidents to Boise State Risk Management, regardless of severity. Incident/Accident reports will be reviewed by Risk Management to determine if a claim should be submitted under the Accident Medical policy.
Program Directors/Coordinators should follow up with the participant’s parents to determine the participant’s injury status and advise Risk Management of treatment and injury severity.
If claim submission is warranted, Risk Management will complete Part 1A information on the claim form and forward to the parent(s)/guardian(s) of the program participant for completion and submission to the insurance carriers Third Party Administrator (TPA).

Accidental Medical Insurance Criteria for Claim
Risk Management will provide participant’s parents or guardians with the partially completed claim form for their consideration and submission based on the following criteria:
- Medical treatment: emergency care and/or physician visit occurs;
- A parent or guardian inquires and requests a claim to be submitted for required treatment based upon an injury sustained while participating in a Boise State Youth Program registered with Minors on Campus.
How to Submit a Claim
A parent/legal guardian is responsible for completing the claim form and submitting to the TPA on behalf of a participant in a covered program once Risk Management has forwarded the claim form to them.
Instructions on final submission will be provided in the email accompanying the forwarded claim form.

Should a parent/guardian not have the means to submit the claim online, they may complete the claims process by phone by calling 800-445-3126. Risk Management will submit the partial claim form to the TPA for completion over the phone by parent(s)/guardian(s).
