Access Agreement for a Thesis or Dissertation
Applicant Exception Request
This form allows applicants to request an exception to a Graduate College admission requirement. To submit this request, applicants must have an active account in the application system, with a completed application and paid application fee.
Application for Admission to Candidacy
This form is required for all graduate students before applying for graduation. It allows the Graduate College to verify that all required coursework for the degree has been or will be completed. Students use this form to list the courses they have taken or plan to take toward their program requirements. Detailed instructions for completing the form are available on the Application for Admission to Candidacy page.
Appointment of Supervisory Committee
This form is used to officially designate the chair and committee members who will guide a student through their defense. It is required for all thesis and doctoral students. Detailed instructions for completing the form are available on the Appointment of Supervisory Committee page.
Defense Committee Approval (PDF)
This required form provides the official approval page template for thesis (master’s) and dissertation (doctoral) students. It must be signed by all committee members immediately following a successful defense and submitted to the Graduate College for final approval.
Defense Notification
This required form is used to notify the Graduate College that a graduate defense has been scheduled. It also provides the necessary details to publicly announce the defense.
Embargo Request for a Thesis or Dissertation
This form is used by graduate students who wish to request an embargo on their thesis or dissertation, temporarily restricting public access to their work.
Graduate Appeal Form (PDF)
This form is used by students to request an appeal or exception to a Graduate College policy or regulation. Appeals are reviewed once, and the Graduate College serves as the final authority in the decision process.
Graduate Program Change Request (PDF)
This form is used by students and advisors to request a change from one graduate degree program or track to another within the same field of study.
Request Approval of Transfer Credits
This form is used to request approval for transfer credits to be applied toward the degree requirements listed on the Application for Admission to Candidacy. Detailed instructions for completing the form are available on the Approval of Transfer Credits page.
Request for Adjustment of Academic Requirements
This form is used by students and advisors to request changes to the list of courses approved on the Application for Admission to Candidacy for completion of a graduate degree. Detailed instructions for completing the form are available on the Adjustment of Academic Requirements page.
Request a Graduate Program Advisor Change
Students use this form to request a change of their major advisor.
ScholarWorks Authorization for a Selected Graduate Project (PDF)
This form must be submitted before a graduate student’s project can be archived in ScholarWorks.
