By submitting this OSP Budget Adjustment Request Form, you are certifying and agreeing that:

1. The information and representations are complete and accurate to the best of your knowledge
2. The appropriate programmatic and administrative personnel approve of this request
3. Authority to approve the action(s) requested is subject to the understanding that the general level of effort, scope and the objectives of the approved project will not be altered, and that the proposed action is reviewed in advance and is consistent with the award conditions
4. Authority to approve changes in PIs and changes in objectives or scopes of approved projects is retained by all sponsors
5. It is the PI’s responsibility to determine that funds are available for the re-budgeting request.
