School of Medical Imaging and Radiologic Sciences Application Information

The application for the Diagnostic Radiology, Magnetic Resonance Imaging (MR), Computed Tomography (CT), Diagnostic Medical Sonography (DMS), Interventional Radiology (IR), Interventional Cardiology (IC), Mammography or Vascular Ultrasound programs consists of the materials and information listed below. Details about each step can be found in the next section on this page. All application materials must be received by the school by March 1st at 4:00 PM MST.

Please use this list as a checklist to gather the materials and documentation that applicants will need in order to complete an application to a program. Do not start the application form until the following has been gathered/completed:

  1. Acceptance to Boise State University – Applicants must have applied and been accepted to Boise State University prior to applying to any school programs. Apply to Boise State University online.
  2. Letter of Application / Statement of Purpose – Applicants will save this letter as a PDF document and upload it into the application form when prompted. See details and instructions in the next section.
  3. Work and Volunteer History and Resume – In the application form, applicants will be prompted to list information related to work and volunteer experience, including dates worked, organization worked/volunteered for, the position and responsibilities held. Applicants will also be asked to upload a resume outlining any patient care experience.
  4. Higher Education History – In the application form, applicants will be prompted to list all higher education institutions attended and the dates of attendance. Applicants will also be prompted to list all degrees earned.
  5. Transcripts – If applying to a diagnostic radiology program all transcripts must be submitted and received by the application due date, even if they have previously been submitted to Boise State. If applying to other imaging disciplines all transcripts not previously submitted to Boise State must be received by the application due date.
  6. References – Applicants will be prompted to list two references: a work-related reference and a general character reference. Applicants will provide the names and email addresses for the references when prompted in the application form. References will be sent an email with a link to complete the reference survey.  The reference surveys must be completed and submitted by the application due dateFurther instructions may be found in the next section.

Application Fee â€“ Applicants will pay a $20 application fee for each program applying to. Applicants will be directed to our secure online payment site while completing your application form.

Names

Applicants will be prompted to provide their name and all previous names – such as maiden name or a previous married name. This is to help the school pair up associated applications, references and transcripts.

Applicants will be promoted to provide a preferred phone number. 

Applicants will also be asked if they have applied to any program within our school in the past. 

PLEASE NOTE: It is the APPLICANT’S responsibility to keep the School of Medical Imaging and Radiologic Sciences informed of any changes in mailing address, phone number or Boise State e-mail address. The school may need to contact applicants regarding the application and/or qualifications prior to or after the deadline. If applicants need to update contact information, please email Tara Person at taraperson292@boisestate.edu. Applicants also need to update their contact information in myBoiseState for other Boise State communication.

Acceptance to Boise State

To apply to the Diagnostic Radiology, MRI, CT, DMS, IR, IC, Mammography or Vascular Ultrasound programs, applicants must have previously applied and been accepted to Boise State University. This is a separate application process.

The applicant’s name along with Boise State student ID number and Boise State BroncoMail address (both given to students once accepted to the university) will auto populate into the application once the applicant enters their name. All email communication regarding an application to our program(s) will be sent to the applicant’s BroncoMail. Applicants are expected to check this email account regularly.

An applicant is promoted to select the program(s) applying to and then rank these programs in the order of preferred acceptance.  Applicants may apply for up to three programs.

Remember, applicants will be charged an application fee for each program selected on the application.  Applicants are also required to upload a letter of application / statement of purpose for each program into the application.

The following information is provided to help applicants choose programs based on eligibility and interest:

On Campus Programs

If accepted into an on campus program, students are placed for clinical experiences within the Treasure Valley (The Treasure Valley includes locations around Boise, Nampa, Caldwell, Meridian, McCall, Fruitland, Mountain Home – Idaho or Ontario, Oregon).

Online Programs

Online program students participate in clinical experiences outside of the Treasure Valley.  Applicants to our online programs must work with the AMI online team to find their own clinical placement in their local area.   A clinical site survey document, completed by the imaging department manager of the secured clinical placement, must be submitted with this program application, demonstrating their intent to accept you for clinical rotations. The clinical site survey document must be completely filled out and include any required documentation or it will be returned to you.  If you have questions related to this document, please contact the AMI Application Team Manager at beckyclawson@boisestate.edu.

  • Interventional Radiology/Interventional Cardiology – Certificate (MUST BE CURRENTLY EMPLOYED IN AN INTERVENTIONAL SETTING TO BE ELIGIBLE)
  • Mammography – Certificate (MUST BE RT(R) CREDENTIALED TO BE ELIGIBLE)
  • Vascular Ultrasound – Certificate (MUST BE RT(R), RT(S), RT(NM), RDMS (BR) or RDMS (MSK) CREDENTIALED TO BE ELIGIBLE)

Letter of Application/Statement of Purpose

Formatting for Letter of Application

The application for admission to our programs requires a narrative Letter of Application for each program you are applying for.

Follow the formatting instructions listed below, save your letter as a PDF and upload it to the application form.

Your letter should be typed, double-spaced with 1 inch margins, using Times New Roman 12pt font.

The letter should be no longer than 3 pages in length.

The letter should be addressed to the Program Admission Committee.

Your letter should reflect serious thought.

Items to include (but not limited to):

  • Please describe your motivation for pursuing a career in healthcare.
  • Explain how your interests and life experiences stimulated your interest in medical imaging.
  • Discuss your investigation of the field of medical imaging, specifically the program you are applying to.
  • Discuss how you will maximize your unique abilities to prosper in this program and become successful in the program you are applying to.
  • What short and long term goals do you have for your medical imaging career?

Please attach a cover page with your letter to include your name, but do not put your name in or on the actual letter.

Work and Volunteer History

You will be asked to list your work and volunteer positions, the organization for which you worked and the dates when you worked those positions. You may add a new row(s) to document multiple work or volunteer positions. If there are no work or volunteer positions to document, just type NA in the first row of fields and continue with the application.  You will also be asked to upload a resume outlining any patient care experience.

Higher Education History

You will be asked to list all higher education institutions that you have attended and the dates of attendance. If you earned a degree, please list the degree earned at that institution. Add a new row to enter information for multiple higher education institutions.

You will also be asked to acknowledge that you must submit all transcripts, including Boise State’s, to the school within the application form. Details on how to submit transcripts are included in the application form and in the Transcripts section on this page.

Transcripts

Copies of all transcripts, official or unofficial, including Boise State’s, MUST be included in this application to be properly processed.

Course enrollment for the prior semester should also be included, if currently enrolled.

A transfer equivalency listed on another institution’s transcript will not satisfy Boise State’s requirement of submitting all original transcripts documenting course completion.

Applications are not complete until all transcripts have been submitted and received.

Application deadline: 4:00 PM MST on March 1st.

References

Instructions for References

The names and requested contact information for two (2) references (one work-related and one general character) must be listed on the application.  An email will be sent to the references listed on the application asking them to fill out a reference survey.  These reference surveys must be completed and submitted to the school by the application deadline (4:00 PM MST on March 1).  If the applicant is applying to multiple programs, reference surveys will be used for all the programs selected on the application.

These references are closed, meaning the individual filling out the survey will return the reference survey directly to the school via a software platform. 

References listed on the application cannot be relatives of the applicant and should be completed by individuals who can speak to the abilities of the applicant.

References will be checked.

Applications are not complete until two reference surveys have been received by the school.  Application deadline: 4:00 PM MST on March 1. It is the applicant’s responsibility to communicate this deadline to chosen references.

References Categories

One reference survey should be completed by each of the following type individuals:

  1. A Work-Related Reference should be an individual (non-relative) that has recent knowledge of the applicant’s work ethic or employment abilities. This can be a current or past supervisor, employer, volunteer coordinator, etc.
  2. A General Character Reference should be an individual (non-relative) who knows the applicant well enough to give a recommendation on personal and ethical characteristics of the applicant. This can be a minister, co-worker, coach, etc.

Application Fee

$20 Application Fee

There is a required $20 application fee to the School of Medical Imaging and Radiologic Sciences for each program applied. Applicants must click a link found in the applicant’s payment acknowledgement statement to be directed to a secure online payment site when filing out the application form.

Applications are not complete until the applicant has paid all application fees.  Application deadline: 4:00 PM MST on March 1.

Additional Notes About the Application Form

Awareness and Consent

Applicants are prompted to read a series of statements regarding the conditions and responsibilities of being accepted into a program within the School of Medical Imaging and Radiologic Sciences. In order to progress through the application, applicants must state agreement to each statement. If an applicant has questions about any of these statements, they are asked to please contact Tara Person, Administrative Assistant for the School of Medical Imaging and Radiologic Sciences at (208) 426-1996 between 8:00 AM and 4:00 PM MST Monday through Friday or email Ms. Person at taraperson292@boisestate.edu.

Applicants will receive a confirmation email at their Boise State email address verifying the application has been received.  If the application has any elements, the applicant will be notified via the same email address after the application has been reviewed.

Remember an application is not complete until the school has received:

  1. Application Form
  2. All transcripts
  3. Two completed reference surveys
  4. Paid $20 Application Fee for each program 

All items must be received by the application deadline, 4:00 PM MST on March 1.

Eligibility for Certification

For Your Information:

The American Registry of Radiologic Technologists (ARRT) holds applicants for certification and certified professionals to a high ethical standard.  All applicants and registered technologists must comply with the ARRT Standards of Ethics and Rules of Ethics.  The following are examples of what must be reported to the ARRT:

  1. Charges or convictions of a crime, including misdemeanors or felonies.  This includes judgements that were stayed, suspended, set aside, deferred or withheld.  Includes court convictions and military court-martials.
  2. Traffic violations charged as a misdemeanor or felony.  This includes speeding tickets and parking violations that are charged as misdemeanors or felonies.  All traffic violations that involve drugs or alcohol.  A traffic violation that does not rise to the level of a misdemeanor and does not involve drugs or alcohol does not need to be reported.
  3. Violations of state or federal narcotics or controlled substances laws.  Applicants and registered technologists need not be charged or convicted but are required to report this.
  4. Violations of the honor code at the educational institution applicants/students attended to become a radiologic technologist.
  5. Disciplinary actions involving a professional permit, certification, license, or registration.
  6. Note:  Offenses committed as a juvenile and adjudicated through the juvenile court system do not need to be reported.

The ARRT has the power to deny any credentialing application based upon the above information.  Admission to the School of Medical Imaging and Radiologic Sciences programs and passing program-mandated background check verification does NOT GUARANTEE acceptance by the ARRT or any other credentialing body.
