Graduate Assistant Tuition and Fee Payment Request

Please use the GA Tuition and Fee Payment Request Spreadsheet provided. Download the file as a Microsoft Excel (do not complete the form in google sheets) After you have entered your information in the excel spreadsheet and saved it, attach it below.

Please use the GA Tuition and Fee Payment Request form to submit payment for a single GA.

Please note if the student is taking any courses that are self-support or designated as alternative fee programs, the amount of standard tuition and the amount of the alternative fee must be listed separately in the sections below. For reference, see University Tuition and Fees information, including "Graduate Tuition & Fees" (standard tuition) and the "Alternate Fee Programs" lists.

Click on the drop down option under 'Type' to select the corresponding tuition or fee type then enter amount for the tuition and/or fee and the funding source. If there is more than one type of fee, select yes on the additional fee question at the bottom of this section.
