The FY22 Open Enrollment Period is April 26 – May 14, 2021

There are some great changes coming for the new plan year.

  • Health Insurance Premium Rates:
  • Preventative Dental services will be covered 100% with no deductible required.
    • Employees will only pay the deductible and copayments for Major Dental services.
  • The deductible for the Dental Major services will increase from $25 to $50.
  • Emergency Room (ER) visits will be subject to a $100 copay (PPO & Trad plans).
    • If the visit results in hospital admission, no copay will be assessed.

New plan year information to keep in mind:

  • The Vision benefits have their own Plan document.
    • There are no changes to the FY22 Vision Plan.
  • MDLive visits are covered at 100% through June 30, 2021.
    • As of  July 1, 2021, MDLive visits are subject to copay and coinsurance.
  • The annual contribution for the Dependent Care FSA will increase to $7,746.00 per family.
    • $3,873.00 if reporting separately
  • You may now roll over up to $550 of your unused Health FSA funds to the upcoming Plan Year.
  • All specialty prescriptions will be filled by IngenioRx’s Exclusive Specialty Pharmacy
  • All plans will now include a diabetes lifestyle management program (Livongo) and a weight management program (Wonder Health) at NO COST to members.
  • Other previously excluded coverages will now be covered services including acupuncture, breast reduction surgery, nutritional formula, wigs, diabetic shoes, and orthognathic (jaw) surgeries. Refer to the SBC’s and plan documents for detailed information.

Hard Deadline

All forms must be received by the HR Benefits Team by May 14th. Please consider this if you intend to mail the forms as they must be in our office by May 14th or, if you drop them off our office closes at 5pm.

You can send paperwork via email to, drop it off at the HR office, or back by popular demand is the Drive Up and Drop Off (see below). You can bring your new benefit paperwork and drop it off to HR without stepping out of your vehicle!

Open Enrollment

Open Enrollment is the only time each year that you’re able to:

  • Change medical plans from a Traditional, PPO, or High Deductible plan.
  • Elect previously declined dependent dental coverage (dependent must also be enrolled in a medical plan)
  • Switch premium deductions from post-tax to pre-tax or vice versa.
  • Enroll in the Healthcare Flexible Spending Account (FSA).
  • Enroll in the Dependent Care Flexible Spending Account without a qualifying event.
  • Enroll in or cancel the ARAG Legal Group Plan.
    • Changes to ARAG need to be made at directly with ARAG Legal.

Enrollment in the Healthcare and Dependent Care Flexible Spending Accounts (FSA) does not automatically continue from one year to the next.

Employees who want to participate in the Flexible Spending Accounts (FSA) for the FY2022 must enroll/re-enroll during Open Enrollment. Please note, if you are not making any changes nor enrolling in flexible spending, no action is needed during open enrollment.

If you are making changes, Open Enrollment and FY22 FSA forms need to be received in HR by May 14, 2021. We will not be able to make exceptions for those who miss the deadline.

Pro Tips and Best Practices

We want to make sure that all employees who submit changes to their benefits during Open Enrollment do so with ease and without errors. Make sure to follow all instructions and submit your forms by the May 14th due date. Here are some other helpful tips:

  • You can send paperwork via email to
  • You must fill in your complete address with Address Number, Street, City, State and Zip Code
    • This is a good time to check if your phone number, address and email are updated in the HR Self-Service
  • If you handwrite your forms, please write legibly to ensure proper processing.
  • If a form asks for an Agency the answer is Boise State University
  • Make sure you are filling out forms for FY22 (or only use forms linked from this page).
  • Most Importantly: Be sure to sign the form! We can accept electronic signatures. The digital signature should include your name and date/time stamp to verify the actual date the employee signed the form.

Open Enrollment Forms and Instruction Videos

You may email, fax or mail the form, but choose one method only. If you email or fax the form to us, please do not mail the original. This duplicates the process and creates additional work and possible delays on our end.

No exceptions can be made for those who miss the May 14 enrollment deadline.

Open Enrollment During COVID-19

We understand that not all employees will have access to printers and scanners while working remotely and would prefer to pick up the paper forms at the Human Resource Office. We are offering Drive-by/Drop-off sessions where employees may drive up to the Human Resource parking lot (located in the Capitol Village Buildings at the corner of University Drive and Earle Street). There you can complete and return all Open Enrollment forms and receive answers to any questions you may have about the forms.

Employees will be asked to remain in their vehicles. A Human Resource team member will bring the forms to you. For safety, our staff will comply with the CDC guidelines.

The Drive-by/Drop-off sessions will be held:

  • Wednesday, May 5th, 10 am – 2 pm
  • Wednesday, May 12th, 10 am – 2 pm
  • Friday, May 14th, 10 am – 5 pm – Final day to return forms

Note: If you are not making any changes nor enrolling in flexible spending, no action is needed during open enrollment.

Remember: You can add or delete dependents from medical coverage at any time during the year. This action is not limited to the open enrollment period.

  • In the event an employee experiences a divorce, they need to take action to remove their ex-spouse from their plan(s) as they are no longer eligible for coverage.
  • You can add or delete dependents from medical coverage at any time during the year. This is not limited to the open enrollment period.
  • Dependent Eligibility Verifications are required when enrolling a spouse or children to your health benefits.
    • HMS is the vendor that will request your dependent’s information.  This is not a scam. If you do not reply within the deadline provided in the request, your dependent will lose the insurance coverage.


Do I have to enroll/re-enroll in FSA every year

Yes. Enrollment does not rollover automatically from one fiscal year to another.

How many pay periods do we have in FY22?

There are 26 pay period in FY22.

When is the Benefit Effective Date?

July 1, 2021.

How Can I Find My Hire Date?

    • You can find your Hire Date through the PeopleSoft Self-Service:
      • Log-in to “”.
      • Select “Employee”.
      • Under Services, click on “HR System”.
      • Click on “Human Resources”.
      • Under Benefits, click on “Benefits Summary”.
      • Then click on Related Content on the upper right-hand corner of the screen for your Last Start Date.
