Appointment of Supervisory Committee Form – Use this form to nominate faculty members that will advise your research and evaluate your progress in the PhD program. This form needs to be approved before you take the comprehensive exam.
Application for Admission to Candidacy Form – Use this form to create a detailed plan of study for your degree. Submit this form after you complete the comprehensive exam.
Application for Directed Research Form – Use this form to submit a request to enroll in directed research.
Request for Course Substitution Form – Use this form to request a core course substitution or a track course that was not pre-approved.
Request for Transfer Credit Form – Use this online form to manage transfer credits.

Comprehensive Exam Policy – Use this policy to guide you through the written and oral comprehensive exam process.
Comprehensive Exam Approval Form – Use this form to document the results of the written and oral elements of the comprehensive exam.
Comprehensive Exam Rubric – Each member of your supervisory committee will need to fill out this rubric. Print and bring to your exam.
Affirmation of Original Work Form – Use this form to certify that the grant proposal you submit as part of the written comprehensive exam is your own work..
Report of Failure of Comprehensive Exam Form– This form needs to be submitted to the Graduate College if a student is unsuccessful at passing the comprehensive exam.
Request to Retake the Comprehensive Exam Form – Use this form to formalize the request to re-take one or more elements of the comprehensive exam.

Final Oral Defense Approval Form – The defense committee will use this online form to record results of the oral dissertation defense with the Graduate College.
Final Dissertation Reading Approval Form – Use this form to certify that Final Reading Approval has been given by the supervisory committee prior to the student submitting their dissertation to the graduate college.
