- Application for Admission to Candidacy Form – use this form to create a detailed plan of study listing the courses required for the degree or certificate
Comprehensive Exam Forms
- BME Comprehensive Exam Policy (PDF) – use this form to guide you through the oral and written comprehensive exam process
- Affirmation of Original Work (PDF) – use this form to certify that the grant proposal turned in as part of the written comprehensive exam is the student’s own work
- Comprehensive Exam Approval Form (PDF) – use this form to document the passsage or failure of the writeen and oral elements of the comprehensive exam
- Report of Failure of Comprehensive Examination (PDF)– submit this form to the Graduate College immediately after a student has failed the comprehensive exam.
- Request to Retake the Comprehensive Exam Form – use this form to formalize the request to re-take one or more elements of the comprehensive exam
Dissertation Forms
- Appointment of Dissertation Committee Form (PDF) – use this form to identify the members of the dissertation committee
- Proposal Acceptance Form (PDF) – use this form to certify the dissertation proposal has been accepted by the dissertation committee
- Final Oral Defense Approval Form (PDF) – use this form to prepare the approval page to take to the oral dissertation defense. Submit to the graduate college upon completion
- Final Dissertation Reading Approval Form (PDF) – use this form to document the signed Defense Committee Approval and Final Reading Approval
Additional Forms
- Application for Directed Research – use this form to enroll in directed research that is not part of your dissertation
- Course Plan (PDF) – use this form to map out your path to a Ph.D. in Biomedical Engineering
- Progress Report (PDF) – use this form to ensure the student is making satisfactory progress towards the degree. Complete this form with your research advisor at the conclusion of each spring semester
- Request for Course Substitution – use this form to request a core course substitution or a track course that was not pre-approved
- Request for Transfer Credit – use this form to change how a transfer course was reviewed and equated
- Academic Adjustment – use this form to to waive a graduation requirement or substitute a transferred course for a graduation requirement.
