Research and Creative Activity Hub Forms
These forms are intended for use by Principle Investigators and staff who have sponsored projects administered by the COAS RCA Hub. Forms submitted will be routed to the appropriate assigned Research and Creative Activity Administrator for processing.

| Personnel Requests | Employee Data Change Use this form to request changes to existing employee contracts (e.g., funding source change, salary change, separations, end date extensions). |
| Travel and Reimbursement | Travel Request Use this form to provide details regarding upcoming travel so that your RCA Administrator can submit a travel authorization on your behalf. |
| Travel and Reimbursement | Reimbursement Request Use this form to request reimbursement for a recently completed travel or purchases made with personal funds. |
