- *IMPORTANT* Your business MUST meet the State of Idaho 3rd Party insurance requirements and list State of Idaho and Boise State University as additionally insured. EMAIL A COPY to cliffstarman@boisestate.edu
- Email a copy of your Long-Term Food Establishment Permit to cliffstarman@boisestate.edu
