Insured:

Please bring your insurance card. We will bill your insurance for your test.

Uninsured:

There will be an out-of-pocket cost if you do not have a qualifying insurance plan. Please ask our front desk what our current fees are, these can vary depending on the type of test offered.

Note:

If you have any questions about your insurance coverage, please reach out to your insurance company.

Should your insurance company deny covering the cost of your testing, you will be personally responsible for the testing fee. The purpose of this notice is to help you make an informed choice about whether or not you want to receive these services.
