Academic Adjustment
This form is used to waive a graduation requirement (may require an appeal) or substitute a transferred course for a graduation requirement. This electronic form requires you to log in with your username and password. The form can be initiated by a student or advisor.
Academic Adjustment for Transfer Credit
This form is used to change how a transfer course was reviewed and equated, and to create a transfer rule for that course going forward unless the one-time equivalency box is checked. This electronic form requires you to log in with your username and password. The form can be initiated by a student or advisor.
Academic Appeal Form
NOTE: Please download and complete this form in Adobe Reader or Acrobat.
Please send the completed form with all attachments to registrar-appeals@boisestate.edu. This appeal form is used for adding and dropping classes after the deadlines, past semester complete withdrawals, early reinstatements, most academic adjustments, and changing from audit-to-credit or credit-to audit after the deadlines.
Academic Appeal Plan of Study
This form must accompany an Academic Appeal form for students requesting early reinstatement after a dismissal.
Catalog Year and Program Plan Update (PDF)
This form is used to change a student's catalog year or program plan used for graduation requirements.
Credit for Prior Learning Form (PDF)
This form is used to obtain academic credit when a student challenges a class, has passed a higher level class and wants the prerequisite(s) for that class, or has a portfolio showing competency in a class.
Diploma Replacement
This form is used to order and pay for a replacement diploma for a degree or certificate previously earned at Boise State.
Directed Research Application (PDF)
This form, with the specific approvals, is required when a student wants to be enrolled in a Directed Research course.
Education Abroad-NSE Course Approval
This form is used by students preparing to study abroad in the National Student Exchange (NSE) program.
Enrollment Verification Letter Request
This form is used by students to verify enrollment at Boise State for employers, insurance, loan deferment, and other entities.
Exceed 21 Credit Hours Request (PDF)
This form is required for any student intending to enroll in more than 21 credits. The student must gain advisor approval to enroll in over 21 credits.
Grade Exclusion—Graduate
This form is used to remove a term or terms from the GPA when the graduate student meets the specific requirements.
Grade Exclusion—Undergraduate
This form is used to remove a term or terms from the GPA when the student meets the specific requirements.
Graduation Application Update Request (PDF)
If you have already submitted a graduation application, and need to update your graduation term, catalog year, major, minor, or certificate, use this form to update your graduation application.
Graduation Verification Letter Request
This form is used by students who need a letter confirming their eligibility to graduate for entities like employers or immigration.
Idaho Residency Determination Worksheet
This form is used by students who are trying to establish residency for tuition and fee purposes and must accompany the required documentation listed.
Independent Study—Graduate (PDF)
This form, with the specific approvals, is required when a graduate student wants to be enrolled in a Graduate Independent Study.
Independent Study—Undergraduate (PDF)
This form, with the specific approvals, is required when an undergraduate student wants to be enrolled in an Independent Study.
Information Update
This form is used when students would like to update date of birth, social security number or gender. Student name updates are now on a separate form below.
International Student Drop Form (PDF)
This form is for international students who intend to drop a course.
Permit for Seniors to Take Graduate Courses
This form is required when an undergraduate student enrolls in a graduate-level course.
Reading and Conference Application (PDF)
This form, with the specific approvals, is required when a student wants to be enrolled in a Reading and Conference course.
Receiving Credit for English Composition Based on Test Scores
With this form, students can receive test credit for Engl 101 and/or Engl 102 based on satisfactory ACT/SAT scores. The form can be initiated only by a student.

Receiving Credit for English Composition (Initial Enrollment Prior to Fall 2013) (PDF)
With this form, a student who had enrollment prior to Fall 2013 can receive test credit for ENGL 101 and/or ENGL 102 based on ACT/SAT/Compass results.
Registration Override
This form is used when students would like to add or drop courses they are unable to do on their own due to system restrictions. Late adds must accompany an Academic Appeal Form.
Release of Information (PDF)
This form must be filled out when students would like to give the university permission to give out information protected by FERPA law.
Request to Exceed Maximum Course Registration (PDF)
This form is used when students are attempting to retake a course after the second attempt, requires an academic advisor and department chair signature.
Request to Repeat a Graduate Course (PDF)
To ask to repeat a graduate course, a student submits this form to the Registrar's Office after approval from the program coordinator.
Request to Withdraw from a Class After the Deadline
This form is used when a student requests to withdraw from a class after the last day to receive a W for each session. For session deadlines, see the Academic Calendar.
Second Degree Prerequisite Transfer (PDF)
This form is used for second degree seeking students who work directly with the department of their degree to use prior courses as prerequisites for their current program.
Student-Athlete Drop
This form is used by student athletes who need to drop a course during the current semester.
Student Name Update
This form is used when students would like to update preferred name, primary name, or username.

Academic Adjustment

Academic Adjustment eForm Security
This form requests a change in who can review an electronic academic adjustment or transfer credit adjustment. If you are not the chair or dean of your program, this will need to be authorized before the change is made.
Classroom or Computer Lab Classroom Reservation Request
This form is used to schedule one time events in general assignment academic and computer lab classrooms.
Department Memorandum
This form is used by a department or program head to make program requirement changes to prior catalogs. Changes to future program requirements must be done with a curriculum change request.
Faculty Initiated Withdrawal
Faculty may drop a student from a course for failure to attend class. Forms must be completed and submitted to the Registrar’s Office by the “Last date to Register or Waitlist Without Permission Number” deadline. Date varies by session, see the Academic Calendar for details - 5th day of class for Regular Session courses. See policy #4190.

Honor Society, Student Organizations, and Groups Query Request--Student Involvement and Leadership Center
This form is used by student organizations to submit a formal query request for student data.
Recommendation for Awarding a Posthumous Degree or Certificate (PDF)
This form is used by academic departments to recommend the awarding of a posthumous degree or certificate.
Request a Change in Access to Permission Numbers
This form is used by academic departments to request a change in who can access a department's permission numbers.
Request for Additional Permission Numbers
This form is used by academic departments to request additional permission numbers for a class.
Request for an Exception to the Standard Meeting Times/Days
This form is used by academic departments to request an exception to the standard meeting time policy for classes that are unable to be scheduled within the standard times.
Schedule Change Request for Fall/Spring
This form is used by academic departments to make changes to an existing class/section, add new class(es) or cancel class(es). This form is to be used once the class schedule has been published on the web.
Schedule Change Request Form for Extended Studies (Summer Sessions, eCampus, and Regional Sites)
This form is used by academic departments to add, cancel, or make changes to Extended Studies classes.

Student Records Query Request—Military Recruiters
This form is used by Military Recruiters to submit a formal query request for student data.

Undergraduate Catalog Request
This form is for departments to purchase copies of the current undergraduate catalog for departmental use.
Workshop Request
This form is to request the set up of individual subject workshops or workshops to be combined with other subjects.
