Academic Adjustment

This form is used to waive a graduation requirement (may require an appeal) or substitute a transferred course for a graduation requirement. This electronic form requires you to log in with your username and password. The form can be initiated by a student or advisor.

Academic Adjustment for Transfer Credit

This form is used to change how a transfer course was reviewed and equated, and to create a transfer rule for that course going forward unless the one-time equivalency box is checked. This electronic form requires you to log in with your username and password. The form can be initiated by a student or advisor.

Academic Appeal Form

This appeal form is used for adding and dropping classes after the deadlines, past semester complete withdrawals, early reinstatements, most academic adjustments, and changing from audit-to-credit or credit-to audit after the deadlines.

The Plan of Study form must accompany a Reinstatement Appeal Form for students requesting early reinstatement after a dismissal.

This form is used to change a student’s catalog year or major, emphasis, minor, or certificate used for graduation requirements.

Credit for Prior Learning

This form is used to obtain academic credit when a student challenges a class, has passed a higher level class and wants the prerequisite(s) for that class, or has a portfolio showing competency in a class.

Diploma Replacement

This form is used to order and pay for a replacement diploma for a degree or certificate previously earned at Boise State. Payments can be made on the Registrar store.

This form, with the specific approvals, is required when a student wants to be enrolled in a Directed Research course.

Exceed 21 Credit Hours Request (PDF)

This form is required for any student intending to enroll in more than 21 credits. The student must gain advisor approval to enroll in over 21 credits.

GED College Ready+Credit Exams Form

This form is required for a student to receive college credit from a GED College Ready+ Credit (GED+) exam if they scored between 175 and 200 on a test subject.

This form is used to remove a term or terms from the GPA when the graduate student meets the specific requirements.

This form is used for second degree seeking students who work directly with the department of their degree to use prior courses as prerequisites for their current program.
