School of Medical Imaging and Radiologic Sciences Application Information

Have you previously been dismissed or removed from an {Allied Health} program at Boise State or another institution?

This question on the application addresses past disciplinary actions that led to removal or dismissal from an institutional program.
Note: Examples comprise policy violations, student misconduct, substantiated reports of academic dishonesty, and similar occurrences.

Applicants will be prompted to provide their name and all previous names – such as a maiden name or a previous married name – to help the selection committee pair up associated applications, references and transcripts. 

Note: Updates to primary and preferred names must be processed through the Registrar’s Office.

Applicants will be prompted to provide a preferred phone number, mailing address and physical address (if different from mailing address). 

Note: Applicants are responsible for updating their mailing address, phone number, and BroncoMail address directly in the Student Center of their myBoiseState account to ensure they receive critical communications from the School of Medical Imaging and Radiologic Sciences and Boise State University.

An application to Boise State University is a separate application process. For those who have not yet completed this process, please do so prior to submitting your application to any School of Medical Imaging and Radiologic Sciences program.

Students admitted to Boise State will be given a student ID number and Boise State BroncoMail address. All email communication regarding an application to our program(s) will be sent to the applicant’s BroncoMail. 

The following is provided to help applicants choose programs based on eligibility and interest. Programs within the School of Medical Imaging and Radiologic Sciences (SMIRS) are highly competitive.  Please note that application to a SMIRS program is not a guarantee of acceptance. All program admission decisions will be finalized by a selection committee.

The application for admission to a SMIRS program requires a separate narrative Letter of Application/ Statement of Purpose for each program you are applying for.

Follow the formatting instructions listed. Save your letter as a PDF and upload it to the application form. Letters of Application / Statements of Purpose submitted that do not follow the formatting instructions will not be reviewed by the selection committee, therefore rendering an application incomplete.

Your letter should be typed, double-spaced with 1 inch margins, using Times New Roman 12pt font.

The letter should be no longer than 3 pages in length and include page numbers

The letter should be addressed to the School of Medical Imaging and Radiologic Sciences Selection Committee.

Your letter should reflect serious thought. 

Items to include (but not limited to):

• Please describe your motivation for pursuing a career in healthcare.
• Explain how your interests and life experiences stimulated your interest in medical imaging.
• Discuss your investigation of the field of medical imaging, specifically the program you are applying to.
• Discuss how you will maximize your unique abilities to prosper in this program and become successful in the program you are applying to.
• What short and long term goals do you have for your medical imaging career?

To maintain an anonymous review process, do not include your name or any personally identifying information in your Letters of Application / Statements of Purpose. The Selection Committee will evaluate all statements anonymously during the scoring process.

You will also be asked to upload a one-page professional resume and include the following: 

• Name, address, phone number
• Education 
• Credentials or Certifications 
• Work Experience
• Additional personal or professional skills

Note: You will be asked to re-enter your Work History and Education into those respective sections of the application. Both your current resume and a completed application are required.  

You will be asked to list your work and volunteer positions in this section of the application. Enter the organization for which you worked and the dates when you worked those positions. You may add a new row(s) to document multiple work or volunteer positions. If there are no work or volunteer positions to document, just type NA in the first row of fields and continue with the application.  

You will be asked to list all higher education institutions that you have attended and the dates of attendance. If you earned a degree, please list the degree earned at that institution. Add a new row to enter information for multiple higher education institutions.

Note: The online-AMI Program requires an associate degree or higher from an accredited institution for admission consideration. If you are completing your A.S. degree through Boise State’s Online Degree Pathway (ODP) or if you earned your degree at another accredited institution, please enter that information in this section of the application along with your projected or completed graduation term and institution.

Campus Radiologic Sciences 

• Submit unofficial copies of all higher education transcripts with your application by the deadline, including your Boise State transcript showing current semester enrollment.

Note: A transfer equivalency report is not accepted in place of a transcript.

Online AMI

• If you have taken new coursework since your initial Boise State application, attach unofficial copies (including Boise State with current semester enrollment) to your application and order  official copies sent to the university by the deadline.
•  Official transcripts previously submitted to Boise State do not need to be resent.

Applications will remain incomplete until all required transcripts are received.

Categories

1. A Work-Related Reference should be an individual (non-relative) that has recent knowledge of the applicant’s work ethic or employment abilities. This can be a current or past supervisor, employer, volunteer coordinator, etc.
2. A General Character Reference should be an individual (non-relative) who knows the applicant well enough to give a recommendation on personal and ethical characteristics of the applicant. This can be a minister, co-worker, coach, etc.

Both (2) reference forms must be submitted to the school by the application deadline of March 1st. 

These references are closed, meaning the individual filling out the form will return it directly to the school; the applicant does not have access to the completed reference form.

References cannot be completed by relatives of the applicant and should be furnished by individuals who can speak to the abilities of the applicant.

References will be checked.

An automated email will be sent to your recommenders as soon as you submit your application. 

Note: Enter ONE recommender’s name and email address per field in each category of reference. If multiple names and email addresses are entered into one field the reference form email will not autosend to any recommender and this could cause a delay in completing the application by the deadline.

Applicants will pay a non-refundable $20 application fee for each program applying to. Application does not guarantee acceptance. Applicants will be directed to our secure online payment site while completing your application form.

Note: Online AMI applicants may only select and pay for their primary choice of discipline on the application.

Required only for online pre-AMI students completing their clinical experience outside of the mapped Treasure Valley area. Completed clinical site surveys must be submitted for review through the Google Form found in the Canvas AMI Program Resource Site (AMI PRS), Please read through the process for review thoroughly in Module 10.02
Note: While this can be submitted separately from the main program application, a decision for admission cannot be made until the site survey is received and approved by the respective Program Director, by the March 1st application deadline.

Each national credentialing body holds applicants for certification and certified professionals to a high ethical standard. The following are examples of prohibited conduct that may impact credentialing eligibility:

1. Charges or convictions of a crime, including misdemeanors or felonies.  This includes judgements that were stayed, suspended, set aside, deferred or withheld.  Includes court convictions and military court-martials.
2. Traffic violations charged as a misdemeanor or felony.  This includes speeding tickets and parking violations that are charged as misdemeanors or felonies.  All traffic violations that involve drugs or alcohol.  A traffic violation that does not rise to the level of a misdemeanor and does not involve drugs or alcohol does not need to be reported.
3. Violations of state or federal narcotics or controlled substances laws.  Applicants and registered technologists need not be charged or convicted but are required to report this.
4. Violations of the honor code at the educational institution applicants/students attended to become a radiologic technologist.
5. Disciplinary actions involving a professional permit, certification, license, or registration.
6. Note:  Offenses committed as a juvenile and adjudicated through the juvenile court system do not need to be reported.

These credentialing bodies have the authority to deny any credentialing application based upon the above information.  Admission to the School of Medical Imaging and Radiologic Sciences programs and passing program-mandated background check verification does not guarantee acceptance by the professional credentialing bodies.

Applicants may request an ethics review directly from the credentialing body prior to program application or exam registration. However, approval through an ethics review does not guarantee admission into a School of Medical Imaging and Radiologic Sciences program.
