Use this form to report lost, missing or stolen Boise State University equipment.


Disposal of Excess Inventory Items

Information on how to dispose of excess inventory and assets is available on the OIT Surplus Forms website. This includes:

  • Donation
  • Sale
  • Trade-in

Fabricated Equipment

If you are constructing fabricated equipment, please submit the Fabricated Equipment Questionnaire. For details on what qualifies as fabricated equipment, and for additional information, please visit the Fabricated Equipment page. The Fixed Asset team will review your submission and, if applicable, assign a Work-in-Progress (WIP) number.


Asset Transfer Request

Please complete this form to:

  • Notify Fixed Assets that a tagged asset has moved within your department (new building and/or room number) or requires a new user assignment.
  • Notify Fixed Assets that you are transferring ownership of a tagged asset to another department (including updates to custodian, building and/or room number, user, etc.).

Tagged Asset Warranty Replacement Notification

Please fill out this form if:

  • A newly tagged asset is sent back to the vendor for repair (and possible replacement).
  • A newly tagged asset fails and is replaced by the vendor on site.
  • A tagged asset is replaced under warranty by the vendor at zero or minimal cost to the university (shipping/installation).
