WAIVER OF LIABILITY AND INDEMNIFICATION: I agree to INDEMNIFY AND HOLD HARMLESS Boise State University, the State of Idaho, and their respective officers, agents, employees, and volunteers from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees, brought as a result of my involvement in any activities of the Osher Institute other than classroom presentations including, but not limited to, field trips, Special Interest Groups, outdoor events, hazardous or increased-risk activities, and any transportation that may be associated with these activities.

PHOTOGRAPH AND MEDIA RELEASE: The Osher Institute reserves the right to take photographs or video recordings during any activity. I understand that these photographs and video recordings are the property of Boise State University and I hereby grant my permission for the use of my associated image, voice, or likeness for educational, advancement, or promotional purposes.
