Register using this form if the University is operating or sponsoring the program or activity and the University is assuming responsibility for the Custody and Control of the minor participant(s) – whether on or off University-owned or –controlled property.

Program Information
Pursuant to Boise State University Policy 12060 (Protection of Minors) (“Policy 12060”), all University and Non-University Programs that assume custody and control over minors participating in the program or activity must register the event with the Office of Institutional Compliance and Ethics by submitting the appropriate form below. The Office of Institutional Compliance and Ethics maintains an online database of University and Non-University Programs to ensure the safety and protection of minors in compliance with Policy 12060.
Definition
Custody and Control under Policy 12060 is defined as:
Accepting supervision over, and responsibility for, minors in the absence of their parents or other legal guardians. If no form of contract, waiver, or written assumption of supervision is entered into with a minor’s legal guardian there is a presumption that the University Program or Non-University Program has not assumed custody and control over a minor.
