Register a Program or Activity Involving Minors
Pursuant to Boise State University Policy 12060: Protection of Minors (“Policy 12060”), all University and Non-University Programs involving Minors where the Custody and Control of the Minor participants is assumed by the University or Non-University Program must be registered with the Office of Institutional Compliance and Ethics by submitting one of the following forms below. The Office of Institutional Compliance and Ethics will keep an online database of these University and Non-University Programs, which will be used to ensure compliance with Policy 12060 in an effort to ensure the protection of Minors.
Custody and Control is defined as:
Accepting supervision over, and responsibility for, Minors in the absence of their parents or other legal guardians. If no form of contract, waiver, or written assumption of supervision is entered into with a Minor’s legal guardian there is a presumption that the University Program or Non-University Program has not assumed custody and control over a Minor.
A University Program is defined as any program or activity the University operates or sponsors where the responsibility for Custody and Control of Minor participant(s) is assumed by the University operated or sponsored program or activity, whether on or off University-owned or –controlled property.

A Non-University Program is defined as any program or activity held on University property involving Minors that the University does not operate or sponsor where the responsibility for Custody and Control of the Minor participant(s) is assumed by the Non-University Program or activity.
